Agreement Statement: I hereby agree to permit Pryor Patchers Quilt Guild to display my Quilt during the Pryor Patchers
Quilt Show. I understand that no responsibility for loss or damage to my entry, is assumed by the Pryor Patchers Quilt Guild
or by any of the members. I also agree that any visitor, news media person and/or member may photograph my quilt entry
while it is displayed in the Pryor Patchers Quilt Show; however, any use of said photograph for commercial or personal profit
must have my written permission.

Quilt Entry Rules:
1. The completed Entry Form must be received by Pryor Patchers Quilt Guild Committee NO LATER THAN
OCT. 1st, 2011.
Entry forms will not be accepted after this date without the consent of quilt show Coordinators.
2. Each entry may be entered in one category only
3. All entries must be clean, free of smoke odor or animal fur, and must be constructed of fabric & quilted.
The Quilt Show Committee reserves the right to make final decisions if a quilt does not meet rules and requirements.
4. In each category – First, Second and Third place ribbons will be awarded by the judge. Each first place item will be
eligible for BEST OF SHOW.
5. The Quilt Show Committee reserves the right to make the final decision as to the proper category for each entry.
6. Any quilt that has been entered previously in a Pryor Patchers Quilt Show is not eligible to be entered again.
7. Security will be provided during the quilt show.
8. All quilts must have the entrant’s fabric identification label sewn securely to the back of the quilt giving the owner’s
name.The purpose of the this label is to ensure that the quilt may be returned to the correct individual.
9. A photograph of each quilt must accompany the quilt entry form. Please include your name on the back of the photo.
Quilt entry forms and photos will not be returned to the entrant.
